The Tu-22M3 long-range bomber crew was making preparations for a training mission when the pilots were ejected from the plane.

As a navy Plane Captain one of my jobs was to secure the cockpit of the F-4 Phantom after the pilot had safely landed his plane on the flight deck of the USS Kitty Hawk CV 63 and stepped out of the cockpit. One time a CAG (Carrier Air Group) Officer got out of the cockpit of his plane and had not pinned his ejection seat so as to disarm the ejection seat. I went to get into the cockpit and do my post flight inspection and turnaround of the plane and noticed that the ejection seat was not pinned and was armed and ready to go off. I hesitated and went and got a Safety Officer to inspect it and he told me that I had done the right thing by not climbing into the cockpit with an armed ejection seat and getting ahold of him, he took over from there. I was an Airman with the rank of E-3 at the time this occurred, he took over from there, I could’ve been blown sky high if I hadn’t noticed that the ejection seat was still armed if I had gone about doing my routine turnaround of the cockpit. And later I heard thru the grapevine that the top brass had quietly reamed in a private meeting that Commander a new asshole for being careless like that. He not only could’ve endangered his own life but mine as well for being careless like that. Thank God safety above everything else was so well stressed by the Navy that I instinctively knew what to do in that situation.

    Something tells me it wasn’t a mechanical failure, but instead some NFG saying “Hey, what does this lever do?”
