This is a kind of a major screw up.
I think someone’s career might take a bit of a setback here.
This business of failure to notify affected people of scheduled training exercises that are designed to look as much as possible like a real emergency is all too common. Planners of this kind of stuff ought to have the absolute and mandatory requirement of full notification hammered into them. Literally, if possible.
