Could it be that he didn’t have his District of Columbia Ladder Operator Class C license, but only the Class A which limits the operator to no higher than the 2nd rung?

Tragically, the young man later tried to walk down an up escalator, and he finally collapsed and died after six days.
And to think I used to ‘climb’ various types of ladders back when I focused lighting instrument in the theatre…when I just could have lifted a ladder and knocked the instrument back and forth until the Lighting Designer was satisfied with the focus.
What a time-saver, huh?
