A Freight Conductor riding in a Caboose could have spotted the problem and slowed or stopped the train before any track-side detectors. But that would employ another person and cut into profits.
In the old days, didn’t train guys do train maintenance?
My Grandfather was a train dude and he took maintenance very seriously.
He wouldn’t even get in to drive the train until he had done a thorough check (not wheel bearings, of course, but all that other shit).
